Case management appointments at CCH have moved to an online process. Watch the video and complete the form below to sign up for a case management appointment.

If you need help applying for housing or understanding housing options, please review the drop-downs below for specific information for families or single adults/couples.  Note: These programs do not provide shelter, motel/hotel vouchers, or any immediate housing.

HOLIDAY CLOSURE INFORMATION: 

  • Intake for new appointments for single adults and couples will be closed next week, Monday, December 22 - Friday, December 26. Integrated Case Management will be reopening on Wednesday, December 31st. Click here to access the form on Wed, Dec 31: Integrated Case Management Sign-Up

  • Intake for families will be closed for the next three weeks for staff trainings and program improvements. New intakes will reopen the week of Monday, January 12, 2026. Please check back on this page for more information.

We randomly select 20 individuals per week for an appointment. If you are not selected this week, please sign up the next week.
