At the Coalition, we know that substance use can also strain relationships and crucial support networks in people's lives. The stress and trauma of being unhoused can also exacerbate or lead to the development of substance use disorders. Being unhoused is associated with risk factors that make substance use more dangerous due to lack of safe spaces and social connection.
At CCH, substance use recovery is a part of our integrated healthcare approach. We offer a variety of services and supports for your substance use recovery journey and goals, all backed by evidence-based methods. Please review the options below and see what might be right for you.

Fort Lyon
Fort Lyon Supportive Residential Community combines recovery-oriented transitional housing for people currently or formerly experiencing homelessness with peer support, and educational, vocational, and employment services on a beautiful campus in Southeastern Colorado. This program is available for up to two years for you to take time to recover and improve your physical and mental health.

Sage Ridge
The Sage Ridge Supportive Residential Community is a new Denver Metro Area campus which welcomes individuals from across the state who are experiencing homelessness and seeking addiction recovery support. Individuals may stay for up to two years of transitional housing recovery on a voluntary basis.

Downtown Denver
Substance use recovery at the Stout St. Health Center aims to be as low-barrier as possible. A few of the ways to access services include:
- Come to the Stout St. Health Center during regular hours to meet with a provider and discuss Medication Assisted Treatment (MAT)
- Meet with a Behavioral Health Provider for individual therapy for substance use disorder specific
- Visit the daily afternoon MAT Clinic (Monday-Friday, 1:00pm-4:00pm)
