Come to Stout Street Health Center in person on Monday, Tuesday, Thursday, or Friday between 7:00 am and 11:00 am and on Wednesday at 11:00 am. If eligible, you will be assigned to a healthcare team and our staff will help you coordinate the care you need.

 

ELIGIBILITY:

You are eligible to receive care if:

  • You are homeless, living on the street, in shelters, or “doubled-up” with family/friends

  • You are at-risk of becoming homeless, living in any unstable or non-permanent situation

  • You are currently receiving services or housing through the Colorado Coalition for the Homeless

  • You have chosen Stout Street Health Center as your Medicaid medical home

Existing Patients

Call between 8:00 am and 4:00 pm for a same-day appointment, or come in-person at 7:00 am. On Wednesdays, call between 12:00 pm and 7:00 pm, or come in-person at 11:00 am. Or, schedule your appointment online through the patient website.


To access the patient website, you will need a security token, which you can request from a front desk staff member. Through the patient website, you can access all of your health information in one place, schedule or check appointments, view medical records, lab results, and diagnoses, request prescription refills, and email your doctor.
