PATH (Project to Assist in the Transition from Homelessness)

Walk-in hours are Monday & Thursday, 1 p.m. - 4 p.m. and Tuesday & Friday, 9:30 a.m. - 12:30 p.m. (first-come, first-served).
 

The Project to Assist in the Transition from Homelessness (PATH) is a program designed to help people who are homeless and also experience serious mental illnesses. 

The Colorado Coalition for the Homeless in conjunction with the Mental Health Center of Denver, Urban Peak and CHARG Resource Center, collaborate to offer a wide range of services such as outreach; screening and diagnoses; habilitation and rehabilitation; community mental health; alcohol and drug treatment; case management; supervisory services in a residential setting; and housing related assistance.
